PARTICIPANT CONSENT FOR PROCESSING SPECIAL CATEGORIES OF PERSONAL DATA (SO-CALLED SENSITIVE DATA)

I declare that, having reviewed the GDPR information clause for study participants (see below), I give my explicit, voluntary, and informed consent to the processing of my sensitive personal data by the Administrator, i.e., the University of Economics and Human Sciences in Warsaw, to the extent necessary for the study to be conducted by the Principal Investigator and others involved, in accordance with the terms specified in the participant information (Art. 9(2)(a) GDPR), while maintaining anonymity to prevent my identification by anyone other than those conducting this study.

I acknowledge that, to the extent necessary for scientific research and development activities, my sensitive personal data may be processed, which could reveal racial or ethnic origin, political views, religious or philosophical beliefs, trade union membership, as well as genetic data, biometric data for the purpose of uniquely identifying an individual, or data concerning health, sexuality, or sexual orientation. However, the publication of the results of this research and developmental work will not lead to my identification.

I declare that so-called sensitive personal data will be collected through the following research tool: an online survey questionnaire (please specify, e.g., questionnaire, interview, etc.).

In cases where the scientific study involves audio or audiovisual recording, I acknowledge that the processing of my image or voice will not violate my personal rights. Recorded material will promptly be converted from audio or audiovisual to text (transcription), after which the recording will be permanently deleted, and the transcription containing personal data will be anonymized after the study concludes.* *This paragraph applies only if the research objective requires recording.

I have been informed that I have the right to withdraw my consent for the processing of personal data at any time, which I may do in writing to the organizational unit of the project leader, Prof. AEH Dr. hab. Michał Wilczewski, or electronically at m.wilczewski@vizja.pl. Izhave reviewed the information regarding personal data processing.

 

General Data Protection Regulation (GDPR) Information Clause
for Research Project Participants

● The administrator of your personal data is the University of Economics and Human Sciences in Warsaw.
● You may contact the Data Protection Officer for all matters related to the processing of personal data and exercising data-related rights.
The legal basis for processing your personal data is based on Art. 6(1)(a), (e), and Art. 9(2)(a), (j) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and the free movement of such data, repealing Directive 95/46/EC (GDPR):

  1. Your explicit informed consent to process personal data for participation in the study; Providing personal data is voluntary, but refusal will preclude study participation;
  2. The necessity of data processing for a public-interest task in scientific research and development activities (Art. 11(1)(3) in connection with Art. 4 of the Higher Education and Science Act);
  3. The necessity of data processing for scientific research purposes, under Art. 89(1) GDPR, based on the Higher Education and Science Act, which is proportionate to the designated objective, does not undermine the essence of the right to data protection, and provides suitable, specific safeguards for fundamental rights and interests of the data subject.

● We will store your personal data only as long as necessary to achieve the research project’s objectives; afterward, upon the study’s completion, data will be anonymized. Until this time, data that could identify an individual will be stored separately and only be linked to specific personal details if required for the purposes of scientific research or development work.
 Personal data will not be transferred to third parties.
 You have the following rights regarding the processing of your personal data:

  1. The right to withdraw consent at any time, without affecting the lawfulness of processing based on consent before withdrawal. Consent can be withdrawn through a statement (electronically or in writing) to the project leader;
  2. The right to access your personal data, except where doing so may hinder or significantly impair the achievement of the research and development objectives and is necessary for their completion;
  3. The right to rectify your personal data if inaccurate and to complete any incomplete personal data. This right is waived if its exercise would hinder or impair the objectives of the research and development, and if necessary for their completion;
  4. The right to erasure of your personal data, particularly if you withdraw consent and there is no other legal basis for processing;
  5. The right to object to data processing, particularly if we process it for public-interest purposes due to your particular situation. This right is waived if exercising it would hinder or significantly impair the objectives of research and development, and if necessary for their completion;
  6. The right to restrict data processing. This right is waived if its exercise would hinder or impair the objectives of the research and development, and if necessary for their completion;
  7. The right not to be subject to a decision based solely on automated processing, including profiling;
  8. The right to lodge a complaint with the data protection supervisory authority, i.e., the President of the Personal Data Protection Office.
